Advanced MDT Approach

An Innovative Multi-disciplinary Team Approach for Sleep Problems

Vik Veer has set up the most advanced Sleep MDT in the world. He saw that his patients needed more than just surgery and perhaps a few exercises from YouTube. He wanted to help his patients recieve the very best care in the world which would require the help of many different medical professionals each bringing their own speciality to help each and every patient.
He created a completely new way to help his patients - Read more below.

The Problem with Sleep Medicine
As a Sleep Apnoea Surgeon and the head of the Sleep Surgery department at the Royal National ENT Hospital in London, I have dedicated the last 10 years of my career to helping reduce the severity of sleep problems using operations. While I have been successful in reducing the primary symptoms of sleep apnoea, I have observed that my patients continue to suffer from a range of other medical issues caused by this condition, including insomnia, obesity, diabetes, high blood pressure, and restless legs syndrome.

I found myself unequipped to address these co-morbidities as they were outside my field of medicine. Consequently, my patients were often referred to various departments, resulting in a fragmented and inefficient healthcare experience. This lack of cohesive care led to repeated investigations and delays in treatment, ultimately hindering my patients' overall recovery.

Reflecting on my previous experience as a cancer specialist, where a multidisciplinary team of experts manages patients, I envisioned a similar approach for sleep apnoea treatment. Given the complexity of sleep apnoea and its associated conditions, I realised that a collaborative, multidisciplinary approach would be more effective in addressing the diverse needs of my patients.

In response to this need, I established the Advanced Sleep Health Collaborative, a team of specialists from various medical fields who work together to treat sleep problems from multiple perspectives. By conducting comprehensive investigations at the outset and having all patients seen by our specialists simultaneously, we ensure that treatment can begin in parallel and without unnecessary delays.

This integrated approach allows us to provide better, faster, and more efficient care, ultimately returning our patients to health more swiftly. The Advanced Sleep Health Collaborative is a testament to our commitment to holistic and patient-centred care, and we are proud to lead the way in transforming the management of sleep apnoea.

How an MDT works
A Multidisciplinary Team (MDT) approach involves a collaborative effort where multiple medical specialists come together to manage and treat complex medical conditions. The core principle of an MDT lies in harnessing the collective expertise of various healthcare professionals to ensure comprehensive patient care.

Patients are first assessed by the core members of the MDT, which typically includes specialists from relevant fields, such as physicians, surgeons, radiologists, and other allied health practitioners. During this initial assessment, thorough investigations and tests are conducted to gather critical information about the patient's condition.

Following the assessments, the entire team convenes to discuss their findings from the consultations and tests. This collaborative discussion enables the team to evaluate all available information and decide on the most appropriate treatment options tailored to the patient's needs. A detailed report of this discussion, including all the recommended treatment options, is then provided to the patient.

The treatment options are carefully explained to the patient, ensuring they fully understand the proposed plan. The patient is given the opportunity to decide whether they wish to proceed with the comprehensive treatment plan, which will involve the entire multidisciplinary team rather than a single doctor.

One of the significant benefits of an MDT approach is that it ensures holistic and coordinated care, addressing all aspects of the patient's condition. This strategy prevents patients from being passed from clinic to clinic, reducing fragmentation in care and enhancing the overall treatment experience. By leveraging the expertise of various specialists, the MDT approach improves diagnostic accuracy, facilitates better treatment planning, and ultimately leads to improved patient outcomes.

In summary, the MDT approach is a robust and effective way to manage complex medical conditions, providing patients with a seamless, integrated, and comprehensive care pathway.

An Example of How an MDT Approach Works
timeline through the Sleep MDT process The diagram on the right illustrates a path for one patient with sleep problems. This is an example route for this patient, and most people will have a different path. Some may not want surgery at all, and therefore treatment options are provided for patients based on their requirements.

Simulated Case History
John, a 56-year-old executive, felt like his world was closing in around him. His relentless snoring and ongoing weight gain were straining his marriage, causing him to wake up each morning with a dry mouth, headaches, and a deep sense of shame for depriving his wife of sleep. This struggle didn’t end at home. At work, John barely managed; his stamina depleted by day’s end, leading him to doze off in front of the TV, missing precious moments meant to bond with his wife and family.

John’s GP faced a cascade of concerns: unchecked blood pressure, rising cholesterol levels, and the looming threat of pre-diabetes. The exhaustion rendered exercise and diet regimes powerful in theory but futile in practice. The social strains further isolated John; excuses piled up to avoid golfing holidays with friends, while the fear of losing his driving license kept him from seeking the medical help he urgently needed. He felt cornered, knowing he needed to make drastic changes to avoid spiraling into an even deeper crisis.

Social strains further pushed John into seclusion; excuses piled up to avoid golfing holidays with friends, while the fear of losing his driving license kept him from seeking the medical help he urgently needed. He felt cornered, knowing he needed to make drastic changes to avoid spiraling into an even deeper crisis.

Faced with these overwhelming challenges, John reached a pivotal revelation. He had to confront his fears and take definitive steps towards change. Driven by the necessity to reclaim his health, his marriage, and his future, he realised that seeking professional help was not only imperative but could also be the transformative decision he needed. This newfound determination marked the beginning of a critical journey toward restoration and renewed hope.

Our Solution
The timeline above serves as an illustration of our process. Each patient’s journey is unique, requiring bespoke treatment options to address their specific needs. In the highlighted case, the patient underwent invasive surgery; however, not all patients will necessitate surgical intervention. Many may benefit significantly from non-invasive treatment approaches, such as CPAP, lifestyle modifications, or other therapeutic options.

The benefits of our multidisciplinary approach are evidenced by comprehensive assessments, including polysomnography, cardiac MRI, validated questionnaires, and blood test results conducted before and after treatment. These diagnostic tools provide clear indicators of the improvements in sleep quality, cardiovascular health, and overall well-being.

Our medical team’s commitment to individualised care and the utilisation of cutting-edge treatment modalities ensures that each patient achieves the best possible outcome. By restoring sleep and enhancing overall health, our approach helps patients regain their quality of life and long-term wellness.
